New Client Welcome Form

Welcome to the Stonewater Family! In preparation for your appointment we have a few tasks to help expedite your time at our hospital and to prepare us for you and your pet’s visit. We are excited to meet you!

MEDICAL RECORDS: Please contact all previous veterinary facilities to obtain medical records on your pet and forward them to stonewaterpets@gmail.com. We prefer to receive these records with at least 24 hour notice to allow time for the veterinarian to review all medical history and for us to update all reminders for vaccines and testing. When requesting these, please ask for all medical records including lab results and doctor exam notes. You can have them emailed to yourself so you can follow up if they were received or not and then forward them to us. Due to privacy policies, we often are not released information without the direct request or approval from the pet owner. 

WELCOME FORM: Please complete and email your Welcome Form as this will help us prepare your file for your appointment and shorten your check-in time on the day of your visit. If you cannot complete this form in advance, please ARRIVE 15 MINUTES EARLY to complete this information with our Reception team. 

To send your form by email:

1. Select the form that you will be filling out. The form will open up in a new tab.
2. Complete the form.
3. Then save the form to your computer. (Right-click and select “Save as”)
4. Save the form onto your Desktop.
5. Then attach your form to an email with your name and pet’s name in the subject line and email it to stonewaterpets@gmail.com.
